AI is moving fast. Healthcare can’t afford slow governance.


There is a version of artificial intelligence in healthcare that I genuinely believe in. But after years of watching innovation outpace accountability, the cost of moving too fast is no longer merely financial. Inlightened’s 2025 physician survey on artificial intelligence (AI) confirmed what many of us in the clinical risk space already knew. Physicians are willing to adapt, but they are entirely under equipped. More than half of the surveyed doctors are already using algorithmic tools in their daily workflows, yet fewer than one in three feel prepared to leverage those benefits while protecting their patients from inherent risks. Nearly forty percent report that their organizations have no established guidelines for utilization. When institutional frameworks do not exist, practitioners are forced to improvise. In a clinical setting where a documentation error or a biased algorithm constitutes a direct threat to patient safety, diffused responsibility becomes an organization’s massive liability.

The most common objection I hear from health technology leadership is the assumption that guardrails can be built after the product is proven.
